Health and Medical Costs: Where Frugality Has Limits and Where It Does Not
Healthcare costs are both the most important category to optimize and the most dangerous to cut carelessly. The frugality framework here is: eliminate waste without eliminating care.
Generic medications are identical to brand-name equivalents by law. The FDA requires generics to have the same active ingredient, strength, dosage form, and route of administration as their brand-name counterparts. Paying brand-name prices for a genericized drug is pure waste. GoodRx and Mark Cuban’s Cost Plus Drugs frequently deliver significant savings on both generic and some brand-name medications.
HSA accounts are the best tax-advantaged vehicle most people ignore. A Health Savings Account, available with high-deductible health plans, offers triple tax advantage: tax-deductible contributions, tax-free growth, and tax-free withdrawals for medical expenses. Unused funds roll over indefinitely. Maximizing HSA contributions before other investment accounts is often optimal for those in good health.
Preventive care is frugality. Dental cleanings prevent crowns. Annual physicals catch conditions before they become expensive. Exercise, adequate sleep, and not smoking reduce long-term medical spending far more than any negotiation tactic at a billing office.
Negotiate medical bills. Hospitals bill at inflated list prices called chargemaster rates. These are almost always negotiable, particularly for uninsured patients or large out-of-pocket balances. Many hospitals have charity care programs. Medical billing advocates exist for complex cases.
Telehealth for non-emergency care. A telehealth consultation for a minor infection, rash, or common ailment costs $30–$75. An urgent care visit costs $150–$300; an ER visit starts at $500–$1,000. Telehealth is appropriate for a significant percentage of non-emergency care.
Do not cut necessary care. Deferred treatment reliably becomes more expensive treatment. The frugality calculus in healthcare punishes delay more than almost any other category.
